Topic: Root Cause Analysis (RCA)

Root Cause Analysis (RCA) is a process for identifying the root causes of problems and a systematic approach for responding to them. RCA is based on the idea that effective management should find a way to prevent problems before they occur and affect the work of an entire organization.

RCA is one of the most crucial problem-solving elements in quality management. It is an important part of Lean / Six Sigma methodology, as it is a key component of the analysis phase of DMAIC – define, measure, analyze, improve, control.

There are six major tools of root cause analysis, which are used through the process of identifying the root causes of a problem.

  • Pareto Chart
  • The 5 Whys
  • Scatter Plot Diagram
  • Fishbone Diagram
  • Failure Mode and Effects Analysis (FMEA)
  • Fault Tree Analysis

This month’s discussion will provide an overview of the tools along with an example of applying the tool toward a particular challenge and the outcome/resolution.

Presenter: Jeff Varani

Service Growth Director for New Power Products, Cummins Sales

Jeff Varani is the Service Growth Director for New Power Products at Cummins Sales & Service North America (CSSNA), a division of Cummins, Inc.

Before assuming this current role, Jeff was the General Manager for mid and northern Michigan service operations of CSSNA. Responsibilities included directing the day-to-day operations of shop and field service operations for engine and power generation markets, product sales, customer experience, health & safety, and creating the right environment for the team. His experience leading cross functional teams, and operating knowledge of other market locations helped turn around a key underperforming market.

Prior to moving directly into operations roles, Jeff’s passion for Lean began while he was the Continuous Improvement and Quality leader for Cummins Bridgeway, a legacy distributor that was acquired by Cummins, Inc. in 2014. From 2010 until the acquisition, Jeff built a top talent continuous improvement group that helped introduce and incorporate Lean principles into all 5 business segments that operated within Cummins Bridgeway (OEM Sales, CPP Upfit, Service Operations, Power Generation Rental, and Corporate Functions).

Jeff spent 4 years with Ford Motor Company and was part of the Ford College Graduate (FCG) Finance Program and held various operational finance roles.

Jeff earned both an MBA from Wayne State University - Mike Ilitch School of Business, and a Bachelor of Science in Finance.

Outside of work, Jeff enjoys spending time with his family, playing hockey, endurance sports, and volunteering with Big Brothers and Big Sisters as a Big for his little brother.
